Hey everyone!
I'm running FreeNAS 9.10.2-U4 and my setup includes 3 4TB WD Red Edition (NAS version @5400rpm).
Today I got the following email and a critical error:

CRITICAL: Aug. 1, 2017, 8:09 p.m. - Device: /dev/ada1, Self-Test Log error count increased from 0 to 1

It seems that one of the drives (ada1) is failing.
I took the full details using smartctl -a, but I couldn't quite figure out the meaning of it (of course that I looked it up first, I just couldn't find anything that is sufficiently explained)

Code:
Error 1 occurred at disk power-on lifetime: 91 hours (3 days + 19 hours)
When the command that caused the error occurred, the device was active or idle.

After command completion occurred, registers were:

So it did return a Read Failure, should I be concerned?
If i'm not mistaken this drive is still under warranty, will I be elgible for an RMA on this one?

I'm waiting for your reply,
Dean.

Yes. RMA it.

PS: did you burn in the disk? It would normally take over 91 hours to do it ;)

Dean,
Post the entire output of smartctl -a /dev/ada1 and if that is what you posted above, then use smartctl -x /dev/ada1.

So it did return a Read Failure, should I be concerned?
Yes you should be concerned. Your extended test failed. Yes, if your drive is under warranty then this is cause to RMA it.
