- 6744 reallocated (bad) sectors
- 992 pending sectors
- 992 offline uncorrectable sectors
- No long SMART tests in the log
- The disk's running hot (though that's a more minor concern)
I'd say it's time to replace the disk, but should be well within its warranty period.
