Well in my mind I see the motherboard being married to a single slab of aluminum and it has fins on the outside to help dissipate heat, but if you choose a system with a low heat signature then maybe the case could be somewhat smooth looking or maybe some graphics milled into the case to give it a greater surface area. On the inside of this block of metal would be a few solid pieces of metal which would tower towards the motherboard, specifically hitting the CPU, PCH, voltage regulators, anything which gets very warm to the touch, and lastly the RAM. Things may need to be a bit creative with things like the RAM in order to sandwich them properly and then I'd think worst case situation is to install a small 80mm fan in the case in order to just circulate the air, not exchange the air, and this would spread the heat around the case and keep things reasonable. I'm just thinking outside the box here.
