2x Chelsio T540-LP-CR
1x Chelsio T580-LP-CR

Controllers
H310 mini 1x 256GB Intel SSD for the FreeNAS and 11x 1TB WD SSD for storage

It's unable to remount the root filesystem, and panic'ing. Is the H310 in IT mode, are you using the RAID functionality? Does it panic if you boot from a USB thumb drive?

Hello

Thank you for the quick response

No H310 doesn't have IT firmware and I don't think that need one. All Drives are non-raid mode. H310 support pass trough drives. I have 2 more locations that have H310 controller and I never had problems with them.
I installed FreeNAS on the Intel SSD drive

Thank you

Ok, but that's the chain of events you need to troubleshoot. It can't re-mount the root filesystem, and the kernel is giving up.

Is the boot pool mirrored? If yes, are they roughly identical, no mixing 512 v. 4k sectors, etc... There's something in this setup the kernel doesn't like.

The stock Dell firmware for the PERC H310 has very poor queue depth (25) so you'll want to flash it for that reason alone. Make sure to follow the instructions for the H310 Mini though.

Re: your boot situation - I would suspect failed or bad boot device. Try with a USB stick as suggested by @rvassar

I don't know why, but when I shut down MD1200 I don't get this error. I had failed HDD attached on the MD1200 I told this will be the problem, but it is not. Only when I shut down MD1200 the freenas can load all the way. I have few more LSI with IT firmware I will try them and see if this will fix that issue.
