My point is that I can't reference that new drive. I can't call gpart against it at all. and I don't see it under /dev/

Do you know the serial number of this drive?

Ok, use this script to see if the drive shows up or not, and if yes then the other infos linked to this drive: http://pastebin.com/GXUTK7AZ ;)

I would say you have a dead disk with serial number 5QG31MLP.

Just in case, connect it to another system (Linux, Unix, Windows) and try to run S.M.A.R.T on it there.

I think I know what I did. I removed that drive and test it on another machine and the drive is fine.
I tried to setup the mirror with another drive and I noticed that after I did "gpart destroy ada2" that's when the drive disappeared from the glabel status, but I can still call "gpart create ada2" which added the drive back to the glabel status list. Previously I just assumed i can't call gpart create when I can't see the drive.

Thanks for your help. The mirror is now successfully setup.

It would have been helpful to know that you didn't even try "gpart create ada2", when I asked whether you'd done it. I got the impression you had an error of some sort, when you said "I can't call gpart against it at all."

At least it's working now.

I think I call "gpart create /dev/ada2p1" previously but that failed, that's when I did glabel status and noticed device wasn't listed, which led me to believe the issues are related.
When I tried yesterday I did "gpart create ada2".

So, you didn't use the syntax used in paleoN's referenced message and your's didn't work. Did you add the swap partition?

Code:
    gpart create -s gpt ada0

    gpart add -i 1 -t freebsd-swap -s 2G ada0

    gpart add -i 2 -t freebsd-zfs ada0
