It means the last disk is experiencing failures...
Use smartctl to find out which (glabel status would be helpful as well)

No, zfs was able to recover it...
I'd say to run zpool clear, and if any errors appears again, then yes

Have resilvering done but have some other issues (http://forums.freenas.org/showthread.php?5106-freenas-kernel-lost-device)

Strange thing is I had seatools running for one night (complete scan), and he found no problem.

Edit : Error on setting "native max size" though
