I'm having an issue with my ZFS pool in FreeNAS: http://termbin.com/7rc8

However, I can't seem to figure out what exactly is causing this issue. Here is the SMART output: http://termbin.com/l4za

Is there anything abnormal in there that I'm missing? What exactly is happening? Do I need to replace the drive and if so, is there a specific issue that I can mention when getting a RMA? Or is it okay to just run "zpool clear" and scrub?

If you reboot, the system should clear the errors and bring the drive back online in the pool--until the next time this happens. Two values in the SMART data concern me, the command timeout (#188) and CRC error count (#199). I'm not familiar with the first, but the second often points to a cabling issue. Make sure the SATA cable is securely connected at both ends, and/or try replacing it with another one to see if that makes a difference.

What's your hardware like? Particularly, motherboard, CPU, SATA controller or HBA if any?

Thank you very much for the quick response. I'll double-check the cabling soon. The server is a Lenovo ThinkServer TS140: I'm using the proprietary motherboard, the i3-4330 and the onboard SATA controller. However, I restarted the NAS and the drive still shows as faulted.

Due to the fact that your pool is a two drive mirror, you need to back up your data
if you have not done so at this point (before attempting anything else).
And save a copy of your configuration settings to another computer as well.

I'm not too sure what happened, I just replugged the cable at the exact same points and rebooted and now FreeNAS is resilvering the faulty drive…
