Hi

Upgrading to 11.1 from 11.0-U4 on a Dell R710 failed to initialize the PERC/H200 SAS Controller on boot. The boot sequence was caught in a loop trying to initialize the controller but failing with an IOC Fault 0x4000265e (see screenshot).

Reverted back to 11.0-U4 all working fine.

My System is a:
Dell R710
2 x Intel(R) Xeon(R) CPU X5550 @ 2.67GHz
48GB Memory
H200 SAS Control (2 SSD for cache)
Netapp 4-Port Gigabit SAS PCI-E Raid Controller Card (attach to Netapp Drive shelf)

Please create a report at bugs.freenas.org and post the issue number here. Attach a debug (which will hide the report from others until a dev has a chance to review it).

I have the EXACT same issue with my T610. Has a solution been reached?

Hi Yes, this has been fixed and in the bug report, the H200 card needs to be flashed into IT mode. You should be able to find how to BUT the most importing thing I found out was to Flash the card on the Dell hardware. As I tried on a test rig which worked fine and worked on the test rig but Dell boot checks FAIL.
