TrueNAS server does not reboot to select boot environment (12.0-U5-2)

Hello,
I have noticed that since a couple of upgrades, my server does not reboot to the upgraded version of TrueNAS Core 12.0-Ux.

I have performed today an update to TrueNAS Core 12.0-U5-2 and after reboot, it still shows as running version 12.0-U3 :

In System -> Boot -> Boot environments, I can see that 12.0-U3 is the Active release, and 12.0-U5-2 is set to take effect at Reboot:

However, after running another reboot it does not boot to this version...

When looking at MOTD or uname -a, I get a different version:

I don't see any obvious error message in /var/log/messages.

Any idea what could be the cause of this behaviour and what can be done to resolve it? How can I successfully reboot to the latest installed version of TrueNAS?

I had a similar thing when TrueNAS 12.0-U5 and the watchdog on bootup detected an issue and booted back into the earlier version. Keep an eye on the live console (VGA or serial console) to see what's happening on boot. For my issue it turned out to be my Dell/Intel X710 network card. Can't say for sure if hardware or software, but am leaning towards the latter, as the 12.0-U4 seemed not to have the issue for me.
