As an update to this post as of version 2.1 I'm running pfsense without issue (on 2.2 now). Not sure what my issue was before but port forwarding works perfectly for me now. I also have a static public IP as well from my ISP because I was double NAT'd so it makes me wonder if that was my issue before.

I've found I have to restart pfsense for new firewall/nat rules to take effect. Restarting the firewall service isn't enough.

Weird, I've never had that issue with pfsense. Good to keep in mind though.
