sqlite3.OperationalError when trying to create a new storage pool

I'm trying to upgrade the size of my storage pool, so have removed one pool and am trying to add in a new larger pool, but get a strange error message. I've attached a text file with the entire error message. A screenshot of what I'm trying to add is below. Any help on where to look for a resolution would be highly appreciated!

Hi all, is there anyone that can give me a clue on where to start looking? When you look at the attached text file with the full error message, can you point me in some direction that I can start investigating. Or should I simply install from scratch?
