Future updates should just overwrite your manual changes.
It doesn't quite work like that... boot environments. Means your old boot environment is there unchanged and not overwritten and the new boot environment contains a new copy of the files (which would not contain your changes).

If the issue is not addressed, you will need to apply the "patch" again after updates
You may need to reconsider your patch as the new file may no longer match the lines to remove in the patch.

You would certainly need to make the changes again somehow to keep them.

Will I need to shutdown all my vm's and restart scale for this to work? Never done a patch on scale before

Technically you may be able to just restart the middleware service, but yes you'll have to restart all of the VMs for the patch to affect them, so at that point you mind as well just do a full restart to be safe.

I'd stop all of them, apply the patch and then reboot.
