My configuration is
FreeNAS 8.2
2 LSI SATA host bus adapter one with 16 ports the other with 8 ports
24 hard drives organized in 6 drives of same kind/make per pool with RAIDZ2 and the pools connected together.
The drives are green Seagate and WD hard drives (mix of 2 and 3 TB disks).
I use >1000W power supply.
Today I found that one drive got detached from the system. It appears this occurred at midnight during a ZFS scrub during high I/O load.
My question is: Do you think I have
1) electrical problem with host bus adapter/cable/backplane?
2) hard drive problem?
3) firmware problem?
Because:
1) I use inexpensive NORCO 24 bay server chassis.
2) All 6 Seagate drives (including the one that got lost) have high raw read error and seek error rate but smartctl self test shows no problems with the drives.
3) The controller firmware would need to be updated if I would upgrade to FreeNAS 8.3.
My plan is:
shutdown system, check all cables, check grounding, update firmware on controllers to latest version, reboot, replace disk, resilver.
Do you have any other suggestions?
Addendum I: It turns out the drive has no problems and the SATA port does not recognize hotswap anymore which would indicate controller firmware or electrical problem.
Addendum II: I added cooling fan for LSI controllers and I rebooted the server and all appears to be working again. I have not updated firmware.

I removed the drive (first offline in GUI then physically) and tested it with smartctl on separate computer. It passes a long self test:
The firmware of the controller is 11 and there is version 15 available on LSI website. FreeNAS 8.3 would require firmware 13. I use FreeNAS 8.2 and do not know firmware requirement.
