ISCSI Errors - logical block address out of range

using iscsi to an esxi cluster and I am getting these errors in the console, any ideas what they mean? I noticed these are on DA5, and I just replaced DA5 because it was having write errors. I wonder if the controller could be going out on that port, or maybe the sas backplane?Not sure what to do at this moment as the pool is in the middle of a resilver lol.

I also have a firmware discrepency apparently..

Flash the appropriate SAS2 firmware. Call again if the problem persists.
