Did you replace the disk using the WebGUI?

The drive was probably replaced not using freeNAS. You should offline that drive wipe it and readd it using the FreeNAS gui.

I did do it with FreeNAS, that is the only OS I ran on the server.

I was a long time ago, I may have done it from the CLI
If you used the cli you didn't use freenas. You should offline the drive and replace it using the GUI.

pedantic mode on

It was FreeNAS, just done incorrectly.

pedantic mode off

(Please don't ask me to do -Wall. -pedantic is messy enough).

So I went to the GUI click on the disk but the lower part of the screen where I would select "Offline Disk" is not there. Could this be a weird bug or my install is corrupted?

That usually means you are using Internet Explorer. :)

I've always narrowed it down to Internet Explorer or a plugin/addon for the browser.

IE has no trouble rendering the GUI, last time I checked.

I think it's more likely that that's the wrong screen.

Are you in the "view disks" page? Don't get it confused with the view pool page.

Good call, I was using chrome on my Mac, I used Safari instead and it worked. Thanks

Fowled through, I offlined the drive in GUI then formatted it, Reslivered it - that worked

Code:
[root@freenas] ~# zpool status
  pool: freenas-boot
state: ONLINE
  scan: none requested
config:

    NAME STATE READ WRITE CKSUM
    freenas-boot ONLINE 0 0 0
      da8p2 ONLINE 0 0 0

errors: No known data errors

  pool: storage00
state: ONLINE
  scan: resilvered 2.38T in 7h8m with 0 errors on Thu Jul 30 01:53:27 2015

Internet Explorer is definitely broken. I see it at least twice a week when people call iXsystems support and we have to ask them if they are using Internet Explorer. :P

Never had any trouble with IE11.

I can imagine ancient versions not rendering it properly, though.
