The card has/had firmware version 12 (IT). I'm trying to update it to version 20 (IT). Got the latest firmware and bios from Broadcom/LSI/Avago web site (9201-16e_Package_P20_IT_FW_BIOS_for_MSDOS_Windows.zip). Booted into DOS USB boot drive, ran the commands below and while it erased successfully, the flashing failed with error "Firmware Host Boot Failed!".

Code:
sas2flsh -o -e 6
sas2flsh -o -f 9201-16e.bin -b mptsas2.rom

Any ideas? Can I revert back to the erased firmware somehow and safely get out of advanced mode so the card is functional again? If I have to re-copy the firmware file or obtain a copy elsewhere, can I reboot the computer to try again or will that bricked the card (because I have to remove the boot USB to re-copy the file and that might crash/hang DOS)?

After you erase the old firmware, you must reboot before you can load the new firmware. Rebooting will not brick the card. If you can brick one of them, it must be very difficult to do. I had to go through the process three times before I got it right the first card I flashed.

Why are you using DOS? Use the UEFI flasher.
