CCB errors with USB boot

I've had to re-install FreeNAS (11.1U6) on a new USB flash drive in attempt to restore my FreeNAS installation (original USB boot drive failed), but after successful install I restart and upon first boot I keep getting the errors below and FreeNAS booting stalls (da12 is the USB boot drive). I've tried re-installing on 3 to 5 new and known working USB flash drives and they all produce these errors. Any ideas why this keeps happening and how to resolve?

Setup is Supermicro X8DTL-i mobo, XEON X5550 CPU, 24 GB RAM, 12 mixed drives in various pools, LSI 9211-8i HBA (IT mode), Intel SAS Expander, Chelsio dual port 10 Gbe NIC. USBs are various SanDisk model series USB 2.0 and 3.0 16 GB flash drives.

Do you have a non USB device you can try booting from? Have you tried all USB ports? All signs point to a USB related issue at this point.

I gave up and switched from booting from USB to SSD. I know it's a waste of a SSD, albeit small capacity, but USB booting was becoming increasing unstable over the last couple of FreeNAS releases for me.
