16 X 2TB drives (RAID Z2)
2 X SSD (mirrored for boot)
32 GB RAM
Asus M5A99X EVO MB

This motherboard has 6 SATA 6GB/s and 2 SATA 3GB/s on-board. I have added another PCI-E card with 4 ATA 6GB/s ports.

I wanted to be able to swap in and out a backup drive on one of the 2 slower SATA ports. When I plug the disk in, after a few minutes I get the aforementioned error and the disk never pops up in the disk list.

Here is the whole log:

Is this on Core or Scale?

Is this a hot plug system or are you shutting down to connect the backup drive?

Please identify with detail the backup drive and the PCI-E card you are using.

You report 18 drives and 12 ports - maybe a fully detailed hardware description will help get you some answers.

You brought up a very good point about hot swap. I checked the specs on the motherboard and it looks like I have to enable hotswap specifically for those two ports! The other ports are hotswap enabled by default.

Glad you are chipping away at it...

I see this in your sig:

* 4 SATA 3.0 hot-swappable slots in one PCIe card
* 2 SATA 3.0 hot-swappable slots in another PCIe card

You don't identify those cards, but perhaps you should read this resource https://www.truenas.com/community/r...bas-and-why-cant-i-use-a-raid-controller.139/ , and this, too https://www.truenas.com/community/r...t-multipliers-and-cheap-sata-controllers.177/ just in case your choices have placed you at risk of falling foul of the ills discussed there. These forums are full of tales of woe resulting from inappropriate hdd controller hardware.
