istgt[3135] end of media & lu_disk_lbread() failed errors

I have 2 FreeNas setups nearly identical
HP Proliant G5 with 8GB of RAM.

Difference is in drives

NAS1 has 4 x 460GB Crucial M550 Drives in a ZFS RaidZ1 config
NAS2 has 4 x 750GB Hybrid Seagate SATA drives, RAIDz2 config 1 128GB Samsung SSD as log, 1 128GB SSD Samsung as cache

both have identical config

NAS1 works perfectly - 3 months no crash
NAS2 crashes twice weekly "randomly" or after a heavy transfer

What is the issue and how it can be resolved?

Need moar info!.. Do you get anything in logs about why the system is crashing? Have you done all the applicable hardware checks? RAM? PSU? Bad SATA cables etc?

Based on your thread title are you running through a card where your disks are attached? I know not flashing to the appropriate firmware can cause problems.. That's the best I can do if this is the case..

Yes/Yes/Yes/Yes

Prior to freeNas same machine ran ESX5 for 5 months without any issues

When it crashes I get a screen full of the error posted; but I do not know how to retrieve the log to post
