These are the first errors I have seen with my main array, so a few questons for the more experienced: is this likely the drive or cabling and if it is the drive does this rise to the level of replacement?

Since there was an error in the self test, the drive; not sure though why it still says it is passing smart when a self test failed.

Oh I love a good SMART test mystery! Here are my thoughts based on experience with a few flaky drives over the years, and alot of time spent learning SMART behavior and methodology.

To answer your last question of why its still passing smart, its because the drive hasn't seen enough of any particular errors to cross the threshhold of being declared FAILED by SMART, personal opinion - when SMART status is FAILED, its definitely time to recover ASAP or replace, but there are lots of indicators to look at prior to that to assess health of the drive. So lets dive into your drive specifically:

Relevant events/data I've made note of:
-You failed an extended test at hour 13955 reading from LBA 5680791456
-You have since passed 3 short tests at hours 14182, 14350 & 14413 (this is not surprising as the short test checks <1% of the total LBA's)
-Your attribute data all looks good namely the following are all zero's:
Reallocated_Sector_Ct
Spin_Retry_Count
Calibration_Retry_Count
Reallocated_Event_Count
Current_Pending_Sector
Offline_Uncorrectable

There are 8 errors in the error log, these are what showed up in your console which is what alerted you to the problem I'm guessing. the first 3 occurred at time 14380 and the last 5 at time 14383. All of these are well after the recorded failure of the extended smart test. The LBA's is in question for these errors are:
5790752392, 5790752576, 5790754335, 5639673039, 5660187975, 5660191383, 5768429120, 5779062248

The Sense codes you saw reported by freenas indicate a read error (so that matches what the drive reported) and also indicated it was a medium error.

Personal Opinion:
There is no doubt errors have occurred. Based on the LBA's reported in the error messages and the fact that they are all very close to the LBA reported as failed in the extended test, I suspect you reasonable chance that an area of disk is failing. The failed extended test rules out a cable as being the issue since the test is ran by the drives firmware.

Recommendation:
I'm not entirely sure this drive needs to be written off, I'll explain in a minute. If this drive in a critical pool I would absolutely get a new drive and replace it, if you are comfortable being degraded for a couple days then remove it from the pool and run this drive through a good burnin. For some reason drives sometimes get stuck in this half failed state, and smart wont reallocate, despite them continuing to throw read errors. I had a drive giving me the same issues you were experiencing (failed extended read, read errors and also pending sectors). It was in a non-critical pool so I left it for a couple months to see if it would eventually reallocate and "self-heal" I finally had time so I pulled it from the pool, ran it through a thorough burnin, and its now working with no faults. It will take a couple days to run, but at the end you will either have a drive that you can have reasonable faith in, or know for a fact that its failed. (I'd run 3-4 burn tests on it, and if its stable I'd consider putting it back into service)
