unable to read smart data on one drive

Hello, my boot drive is giving the following errors when trying to read SMART data and it creates an alert. I believe this started when I applied the most recent update although I'm not certain. I've attempted to downgrade but going from 12.0-u6.1 to u5.1 fails (I can post the log if needed). My data drives are connected to the same Intel controller as below and work fine. Anyone have any ideas on the cause of this? I'd be happy to test downgrading if that's possible. Thanks in advance for any assistance.

***editing to say I'm not exactly sure which of the controllers the drive is connected to, so I tested it in all 6 ports and have the same problem. I suppose it's a drive specific error and probably a problem with the drive hardware?
