TreNAS Core Installation Failure

Hello,

I'm trying to perform a clean install of TrueNAS Core 13u4, but it won't get past creating the boot partitions.
Attached is a screenshot of where it stalls out. It will just sit here and do nothing more.

I can figure out what the issue is.
Can anyone help?

Try properly erasing the disks first

CPU: Intel Xeon E5-2680 v3
MOBO: Supermicro X10SRL-F
RAM: Samsung 128GB PC4-2133R (4x32GB)
Boot Drive: Kingston 256GB A400 SATA3 SSD
Data Drives: HGST 4TB 7200rpm SATA3 HDD

Looking more closely at the boot messages - da0 is likley the boot disk. Implying that there is something wrong with your boot drive. I am not saying physically wrong. Some options are:
1. Corrupt
2. UEFI vs (can't remember) boot setup - my favorite
3. Faulty - seems unlikley
