Server Causes IPMI Disconnect During Boot

This post was going to say something about the web UI giving a "404 - Not Found" error, but when I went to the page to get the exact error wording to make this post things got weird and now I am just confused, so I will start from the beginning and hopefully someone here can make heads or tails out of what is happening.

I got my server up and running about 4 years ago with some help from this forum and have had very few problems since then, until now. For starters, I'm running FreeNAS-11.3-U5, and here are my system specs:

If I recall correctly I think I've had one of the 10TB drives fail and get replaced, and currently I have another 10TB drive with bad sectors that I plan to replace soon, but the specific models of hard drives shouldn't matter anyway.

Now for the problem. Last week sometime I had an issue with my internet service being down, and at some point in troubleshooting with my ISP (Verizon FiOS) my router (ASUS RT-AX3000 w/ goCoax MoCA 2.5 Adapter MA2500D) got reset to factory default settings. For some reason it changed the main router IP to something other than 192.168.1.1, which caused its own headaches until I realized that had happened, and obviously factory default means all my static IP and port routing settings were wiped out.

At first I couldn't access anything; no server, no web UI, no IPMI, and no Plex server. Once I got the router settings straightened out I thought it was all good, until a couple of days ago when I lost access to everything again for no apparent reason (except for Plex, which strangely still worked fine). The web UI was giving a 404 error and IPMI was showing the server offline. It seemed like the IP for IPMI got changed again somehow, so once I used the correct IP to connect and rebooted the server it looked OK again, until midway through booting it suddenly disconnected and went back to showing offline in IPMI despite clearly being running.

That was the point at which I decided to come here for help, but as I mentioned at the beginning, when I tried to go to the web UI to copy the error message I was surprised to see it appears to be working fine! Now the web UI, network file access, and my Plex server all seem fine, but I still can't connect with IPMI since it still says offline.

I don't really need to be able to use IPMI as long as everything else is working properly, but I'm concerned I could randomly lose access again so I want to try and figure out what has been going on this whole time and also get IPMI working again if possible.
