Pool Offline after upgrade to 11.3-U5

Hi All,

I recently upgraded from 11.3-U4.1 to 11.3-U5 using the web GUI. The update went on smoothly, however now my pool is Offline.

Please see the screenshots for the system info, zpool status and gpart show.

Any suggestions as to why this could have happened and what can I do to bring it back online? Thanks in advance.
